We wish to make your first visit with us as smooth and relaxing as possible. To lessen the wait time for you, we have made it easy to fill out your patient information forms from home. If you have received your invite to our patient portal via email you can set up your account and your forms will be available. If you have not received your invite, please contact our office at 320-253-4242. If it is not an option to do your forms from home we have a kiosk here to take care of them in office.
